We strongly encourage all patients to confirm with their insurance plan that their chosen clinician is in-network.
For patients whose insurance we do not accept, as well as those who are uninsured or visiting from out of state, we offer competitive self-pay pricing. Please contact your nearest location by phone or email for more details.
As a courtesy, we submit claims to your insurance carrier on your behalf. For the plans listed below, we also submit secondary and tertiary claims. Inclusion on this list does not imply endorsement or certification by the insurance companies.
